گزش خشک در مارگزیدگی زمانی است که علایم موضعی پیشرونده از قبیل تورم، نشانه های سیستمیک و اختلالات آزمایشگاهی دیده نمی شوند و عموما نیاز به تجویز آنتی ونوم وجود ندارد. هدف از این مطالعه، بررسی الگوی گزش های خشک و عاقبت درمانی آن ها در یک مرکز ارجاعی بوده است.
روش هادر این مطالعه ی مقطعی، بیماران با گزیدگی ناشی از مار افعی که از فروردین سال 1392 تا 1398 در بخش مسمومیت های بیمارستان خورشید اصفهان، به عنوان گزش خشک بستری شده بودند؛ مورد مطالعه قرار گرفتند. ویژگی های دموگرافیک، علایم موضعی، سیستمیک و آزمایشگاهی این بیماران جمع آوری و تحت بررسی قرار گرفت.
یافته هادر این مدت، 139 بیمار با گزش مار افعی بستری شده بودند که 47 بیمار (33/81 درصد) گزش خشک داشتند. گزش در مردان شایع تر و بیشتر در فصل های بهار و تابستان و در شمال استان اصفهان و محل گزش مار به ترتیب فراوانی در اندام فوقانی، اندام تحتانی و تنه بود. شایع ترین علامت بیماران دچار گزش خشک، درد محل گزش (63/8 درصد) بود که در 63/8 درصد موارد با داروهای ضد التهاب و در 36/2 درصد بدون هیچ درمان اختصاصی بهبود یافت. در طی بستری و پیگیری پس از آن، بیماران فاقد هرگونه عارضه ای بودند.
نتیجه گیریدر بیماران با گزش خشک ناشی از گزش مار افعی بستری در بیمارستان، درد، شایع ترین علامت بود. با این حال، بسیاری از بیماران دچار گزش مار افعی نیاز به آنتی ونوم پیدا نکردند. این یافته می تواند حاکی از آن باشد که درد محل گزش به تنهایی الزام به دریافت آنتی ونوم را توجیه نمی نماید.

زرین گیاه (Dracocephalum kotschyi)، دارویی گیاهی با خواص ضداسپاسم و ضدالتهاب است. در مطالعات فارماکولوژی، عصاره ی زرین گیاه از بروز التهاب و فیبروز ریوی جلوگیری می کند. علاوه بر این، عصاره ی زرین گیاه، اثر رفع انقباضی روی مجاری تنفسی هم دارد، ولی تاکنون اثرات قلبی- عروقی آن بررسی نشده است. هدف این مطالعه، بررسی اثر اسانس و عصاره ی زرین گیاه روی انقباضات بافت ایزوله شده آیورت و قلب رات در شرایط in vitro می باشد.
روش هااسانس به روش تقطیر و عصاره ی هیدروالکلی به روش ماسیراسیون تهیه گردید. رات های ویستار نر به روش اختناق با CO2 کشته شدند. قلب و شریان آیورت بدقت جدا شده و در حمام بافت حاوی محلول فیزیولوژیک اشباع شده با اکسیژن قرار داده شد و انقباضات آن ها با دستگاه فیزیوگراف ثبت گردید. اثر غلظت های مختلف اسانس و عصاره ی زرین گیاه روی انقباضات قلب و آیورت جداگانه بررسی و با گروه شاهد مقایسه شد.
یافته هااسانس زرین گیاه، اثر مهاری روی انقباضات میوکارد قلبی و شریان آیورت از خود نشان داد. در حالیکه عصاره ی هیدروالکلی زرین گیاه در غلظت هایی که انقباضات عضلات صاف روده را کاملا مهار کرد، تاثیری روی انقباضات میوکارد قلبی یا آیورت نداشت. در غلظت های بالای µg/ml 200 عصاره زرین گیاه اثر اینوتروپیک مثبت روی قلب داشت ولی انقباضات آیورت را مهار کرد.
نتیجه گیریاسانس زرین گیاه اثر وازدیلاتوری قویتری نسبت به عصاره، روی شریان آیورت داشت. برخلاف عصاره ی هیدروالکلی که اثر اینوتروپیک مثبت داشت، اسانس زرین گیاه اثر اینوتروپیک منفی روی انقباضات قلب دارد.

Dry bite in snakebite is when progressive local symptoms, systemic manifestations, and laboratory disorders are not seen, generally there is no need for antivenom. The purpose of this research was to investigate the pattern of dry bites and their treatment outcome in the referral center.
MethodsIn this cross-sectional study, patients with viper bites who were admitted to the poisoning department of Khurshid Hospital in Isfahan from April 2013 to 2019 as dry bites; were studied. Demographic characteristics, local, systemic, and laboratory symptoms of these patients were collected and analyzed.
FindingsDuring this period, 47 patients (33.81% of all snakebites) had dry bites. Bites were more common in men and more in spring and summer, and the north of Isfahan province than in other regions. Snake bites were more frequent in the upper limbs, lower limbs and, trunk, respectively. The most common symptom of the patients was pain at the bite site (63.8%), which improved with anti-inflammatory drugs in 63.8% and without any specific treatment in 36.2% of cases. During hospitalization and follow-up, the patients did not have any complications.
ConclusionPain was the most common symptom in hospitalized patients with dry bites caused by viper bites; However, many snakebite patients did not require antivenom. This finding can indicate that pain at the bite site alone does not justify the requirement to receive antivenom.

Dracocephalum kotschyi is an herbal medicine with spasmolytic and anti-inflammatory properties. Pharmacological studies have indicated that D. kotschyi extract prevents pulmonary inflammation and fibrosis. In addition, D. kotschyi extract had bronchodilatory action. However, so far, its cardiovascular effect has not been investigated. The present study aimed to assess the effect of D. kotschyi extract and its essential oil on isolated heart and aortic contractions in vitro.
MethodsEssential oil and hydroalcoholic extract were prepared with hydrodistillation and, maceration respectively. Male Wistar rats were killed with CO2 asphyxiation. The heart and aortic artery were carefully dissected and placed in an organ bath filled with a physiological solution saturated with oxygen, and their contractions were recorded on a physiograph apparatus. Effects of various concentrations of the D. kotschyi extract and essential oil were investigated on the heart and aortic contractions separately and compared with the control group.
FindingsThe essential oil of D. kotschyi had an inhibitory effect on both myocadiac and aortic contractions. While the D. kotschyi extract in concentration that completely inhibited contraction of the ileum did not affect myocardial contractions. At concentrations above 200µg/ml, D. kotschyi extract had a positive inotropic effect on the cardiac muscle, however, at these concentrations it inhibited aortic artery contraction.
ConclusionThe D. kotschyi essential oil had a more potent vasodilatory effect than the extract on the aortic artery contractions. However, unlike D. kotschyi hydroalcoholic extract which had positive inotropic action, the essential oil had negative inotropic action on myocardial contraction.
